Rules:  Photo must comply with HIPPA rules and cannot contain any patient information.  Contest ends 5/29/15. Entries must be received by 5pm on 5/29/15.  One entry per person.  This contest is for healthcare providers and is limited to Acute Care facilities and their affiliates located in the following states:  NC, SC, GA, AL, FL, and TN.  By registering for this contest, you are giving SeamlessPOC permission to use photo for marketing purposes. Names will not be used without permission.  Before & after photos will be taken by SeamlessPOC technicians for work verification. Email addresses used for submission with be added to an opt-in email list so contest updates can be provided. Winners will need to assist SeamlessPOC in gaining approval/access to work space to perform work.  If you'd like a list of winners, please email stefanie.weathers@seamlesspoc.com after June 5th for a complete list.
